| BACKGROUND |
| To answer to the less comprehensive health care system of the city as per evaluation of the present administration, strategic initiatives were undertaken to expand its reach and coverage. |

Among the strategic initiaitives was the creation of the Hospitalizatrion Card Program, otherwise reffered to as the Blue Card Program. The strategic initiatives include the hiring of the additional health care providers, establishing health clinics in various strategic locations throughout the city, and the creation of the "Botika sa Katawhan." |

The Blue Card Program was designed to provide free hospitalization program to the poorest of the city residents who could hardly afford hospitaliation expenses. It was formulated to provide in-patient services to complement the existing out-patient services of the City Health Office. |

| BLUE CARD BENEFICIARIES |
| Hospitalization in this period of rising pricing prices is not cheap. With limited resources the city cannot offer free hospitalization to all, thus prioritization must be adopted. The poorest of the poor were prioritized as beneficiaries of the Program. The City classified the poorest of the poor as those families who are earning less than 40, 000 pesos annually. These families need 110.00 pesos a day, on the average, to spend for daily basic needs. |

In the latest survey of the City Poverty Reduction Action Program, there are about 2,802 families identified within this income bracket. |

However not all of the 2,802 families can be beneficiaries of the Blue Card Program. One must be of good standing in the community to qualify as a beneficiary. |

| BLUE CARD ADMINISTRATION AND FUNDING |
A Blue Card Office has been created to administer the hospitalizationcard system program. The Blue Card Office will undertake to screen the applicants in accordance to qualifications set by the Ordinance creating the Blue Card Program. The office will also be responsible for the issuance of the Blue Cards to the beneficiaries. It shall implement the systems and processes of administration of the Program as set in the Ordinance. |

| BLUE CARD REQUIREMENTS |
| A photocopy of the following: |

- Marriage Contract ( if married )
- Birth Ceritificate of Applicant and Spouse ( if married )
- Birth Certificate - Qualified Dependents ( 18 yrs. old and below )
- Barangay Clearance
- Community Tax Certificate
- ID Picture / Voter's ID

| PROCESS SEQUENCE |
- Applicants will go first to the Learning Center Office to inquire the requirements and get the application form.
- After completing the requirements, proceed again to the Learning Center office for the requirements evaluation.
- Applicants should proceed to their Punong Barangay for signature.
- The DSWD ( Department of Social Welfare and Development ) will certify if the applicant is indigent.
- Once the DSWD certification is done, the applicant should proceed to Dr. Poricos for signing in the City Health Office.
- Applicants will go to Mr. Mark Monton in the GSO ( Government Service Office ) for signage in behalf of the City Mayor.
- The applicant then go back to the Learning Center Office to get the Blue Card Membership No.
